Overview

This intervention involves the use of rosuvastatin, a small molecule HMG-CoA reductase inhibitor (statin), as an adjunct therapy for HIV-infected adults already receiving standard antiretroviral therapy (ART). Studied primarily in the STAR (Statin Adjunct Therapy Among ART-treated Adults) trial led by Makerere University, the combination aims to mitigate the chronic immune activation, systemic inflammation, and premature immune aging that persist in HIV patients despite viral suppression. Rosuvastatin works by inhibiting the enzyme 3-hydroxy-3-methylglutaryl-coenzyme A reductase, which is the rate-limiting step in cholesterol synthesis, but it also exerts pleiotropic anti-inflammatory effects. In the context of HIV, this adjunct therapy is evaluated for its potential to reduce the risk of non-AIDS-defining cardiovascular and inflammatory complications in patients receiving long-term ART.
