Apalolitovolrelimab (IBI322) is a recombinant humanized bispecific monoclonal antibody that simultaneously targets programmed cell death ligand 1 (PD-L1) and CD47. Developed by Innovent Biologics, the agent is designed to provide dual checkpoint blockade: inhibiting the PD-1/PD-L1 axis to reinvigorate T-cell antitumor activity and blocking the CD47/SIRPα "don't eat me" signal to enhance macrophage-mediated phagocytosis of malignant cells. The bispecific architecture of apalolitovolrelimab is engineered to improve tumor selectivity and mitigate the hematologic toxicities, such as anemia and thrombocytopenia, commonly associated with systemic CD47 inhibition by preferentially binding to cells that co-express both targets. It is currently being evaluated in clinical trials for the treatment of various solid tumors, including non-small cell lung cancer (NSCLC), and hematological malignancies.
