The combination of **apalutamide**, **abiraterone acetate**, and **prednisone** is an investigational regimen evaluated for the treatment of metastatic castration-resistant prostate cancer (mCRPC). **Apalutamide** is an androgen receptor inhibitor that blocks the action of androgens on prostate cancer cells. **Abiraterone acetate** inhibits CYP17A1 (17α-hydroxylase/C17,20-lyase), an enzyme critical for androgen biosynthesis, thereby decreasing androgen production from the testes, adrenal glands, and tumor tissue. **Prednisone** is a synthetic glucocorticoid used alongside abiraterone acetate to counteract mineralocorticoid excess and potential adverse reaction caused by CYP17 inhibition. The regimen acts by targeting the androgen signaling axis in two distinct ways—receptor blockade and synthesis inhibition—and is administered orally. It has been investigated primarily in men who are chemotherapy-naïve and have mCRPC[1][2].
