This combination therapy is a neoadjuvant regimen developed for patients with prostate cancer and limited bone metastases (≤10) who are candidates for radical prostatectomy. It combines **apalutamide**, a potent second-generation androgen receptor inhibitor, with **strontium-89 (89Sr)**, a bone-seeking radiopharmaceutical that mimics calcium and localizes to areas of active bone turnover to deliver localized beta radiation. The regimen also includes a **luteinizing hormone-releasing hormone (LHRH) analog** to achieve androgen deprivation. This intensive systemic and bone-targeted approach is being investigated by Zhujiang Hospital to improve pathological complete response rates and progression-free survival by targeting both the primary tumor and micrometastatic osseous disease prior to surgical intervention.
