**Apamistamab** is a recombinant murine monoclonal antibody targeting CD45, a cell surface antigen broadly expressed on hematopoietic cells but not on non-hematopoietic tissues. It is administered as a radioimmunoconjugate, where the antibody is linked to the beta-emitting isotope iodine-131, forming a radiopharmaceutical agent for targeted myeloablative conditioning before hematopoietic stem cell transplantation (alloHCT). The drug delivers targeted radiation to bone marrow, aiming to eliminate malignant hematopoietic cells while sparing other tissues. Its most advanced development has been for older patients with relapsed or refractory acute myeloid leukemia (AML), especially those considered for alloHCT, and also is in development for other hematologic malignancies and preconditioning prior to CAR-T cell therapy.
