Overview

This therapeutic regimen is a triple combination therapy consisting of apatinib mesylate, camrelizumab, and metronomic capecitabine, currently being investigated by Zhejiang University for the treatment of advanced pancreatic adenocarcinoma. Apatinib is a small-molecule tyrosine kinase inhibitor that selectively targets vascular endothelial growth factor receptor 2 (VEGFR-2), thereby inhibiting tumor angiogenesis and potentially normalizing tumor vasculature. Camrelizumab is a humanized monoclonal antibody that acts as an immune checkpoint inhibitor by binding to the programmed cell death protein 1 (PD-1) receptor, which restores the anti-tumor activity of T-cells. Capecitabine is an orally administered fluoropyrimidine carbamate that serves as a cytotoxic prodrug of 5-fluorouracil (5-FU). In this specific protocol, capecitabine is administered in a metronomic fashion (low-dose, frequent administration) to leverage its potential anti-angiogenic and immunomodulatory properties alongside its direct cytotoxic effects. The combination aims to overcome the highly immunosuppressive and hypovascular microenvironment characteristic of pancreatic cancer.
