The combination of apatinib, carboplatin, and nab-paclitaxel is a treatment regimen that has been studied for advanced pulmonary sarcomatoid carcinoma (PSC), a rare and aggressive subtype of non-small-cell lung cancer. This combination therapy showed limited short-term efficacy in advanced, unresectable PSC cases. In documented cases, albumin-bound paclitaxel (nab-paclitaxel) was administered at 260 mg/m² of body surface area, along with carboplatin (area under curve 5), combined with daily oral apatinib (425 mg). This regimen was used as a first-line treatment for patients who could not undergo radical resection due to advanced staging and who declined immune checkpoint inhibitors for financial reasons. Clinical outcomes from the limited cases showed that two patients achieved a partial response while another showed stable disease lasting for more than 6 months. However, one patient showed disease progression at the 7-month follow-up, indicating the rapid development of resistance to this therapeutic regimen.
