APG-157 + pembrolizumab is an investigational combination therapy being evaluated for the treatment of recurrent or metastatic (R/M) head and neck squamous cell carcinoma (HNSCC). APG-157 is a first-in-class botanical drug candidate developed by Aveta Biomics, consisting of multiple active molecules derived from *Curcuma longa*. It is formulated as an oral thin film that dissolves in the mouth, providing both local topical delivery to the oral mucosa and systemic absorption. APG-157 exerts its effects by inhibiting the NF-kB pathway and reducing the expression of pro-inflammatory cytokines such as IL-1β, IL-6, and IL-8, while also promoting T-cell infiltration into the tumor microenvironment. Pembrolizumab is a humanized monoclonal antibody that binds to the PD-1 receptor, preventing its interaction with PD-L1 and PD-L2 to restore anti-tumor immune responses. The combination aims to transform "cold" tumors into "hot" tumors, potentially overcoming resistance to single-agent checkpoint inhibitors.
