Drug intelligence / Profile preview
Apitegromab + tirzepatide is an investigational combination regimen that pairs apitegromab, a muscle-targeted monoclonal antibody, with tirzepatide, a dual GIP and GLP-1 receptor agonist. Developed by Scholar Rock, apitegromab selectively inhibits the activation of pro- and latent forms of myostatin, a protein that restricts skeletal muscle growth. Tirzepatide, the active ingredient in Eli Lilly's Zepbound, promotes weight loss by mimicking incretin hormones to reduce appetite and food intake. The combination is designed to address a key limitation of GLP-1-based weight loss therapies—the significant loss of lean muscle mass. In the Phase 2 EMBRAZE trial, the addition of apitegromab to tirzepatide therapy over 24 weeks resulted in a 54.9% preservation of lean mass (saving approximately 1.9 kg of muscle) compared to tirzepatide plus placebo, without affecting the overall amount of weight lost or altering the safety profile.
