**Aplitabart + venetoclax** is a combination therapy under investigation primarily for the treatment of hematological malignancies such as acute myeloid leukemia (AML) and potentially other tumors. **Aplitabart** (IGM-8444) is a multivalent IgM monoclonal antibody agonist of Death Receptor 5 (DR5; also called TRAIL receptor 2), intentionally activating the extrinsic apoptosis pathway in cancer cells. **Venetoclax** is a small molecule inhibitor of B-cell lymphoma 2 (BCL-2), disrupting mitochondrial-mediated apoptosis inhibition. The combination targets two separate apoptosis pathways—aplitabart induces extrinsic apoptosis via DR5, while venetoclax inhibits intrinsic anti-apoptotic signals through BCL-2. Preclinical and clinical exploration has focused on AML and related disorders, where dual inhibition of survival mechanisms may overcome resistance in relapsed/refractory disease[2][5][6][7].
