Overview

Aponermin is a recombinant circularly permuted human tumor necrosis factor-related apoptosis-inducing ligand (TRAIL) developed for the treatment of multiple myeloma. It is an engineered peptide mimetic that binds to and activates death receptors 4 (DR4) and/or 5 (DR5) on tumor cells. This activation triggers intracellular caspase reactions via the extrinsic apoptosis pathway, leading to programmed cell death in malignant cells while sparing normal tissues. Aponermin forms stable homotrimers in solution and exhibits higher binding affinity for DR4/5 as well as a longer half-life compared to endogenous wild-type TRAIL. In November 2023, it was approved in China—when used in combination with thalidomide and dexamethasone—for adult patients with relapsed or refractory multiple myeloma who have received at least two prior therapies[1][3][4][9].
