Overview

**Apovir** is an investigational oral antiviral combination of the small molecules **pleconaril** and **ribavirin**, developed by Apodemus for Alzheimer's disease. Pleconaril binds the hydrophobic pocket of enteroviral capsid protein VP1, stabilizing the viral particle and impairing entry and uncoating; ribavirin is a broad-spectrum guanosine analog with antiviral effects that include inhibition of host inosine-5'-monophosphate dehydrogenase and interference with viral RNA replication. The rationale was the hypothesis that enteroviral or other infectious agents may contribute to Alzheimer's disease pathobiology. In a randomized, double-blind Phase IIa study of 69 patients, Apovir did not meet the 9-month ADAS-cog primary endpoint and had poor tolerability, including substantial treatment discontinuation and more frequent adverse events.
