Apraclonidine is a sympathomimetic small molecule used primarily as an ophthalmic solution for the short-term treatment of elevated intraocular pressure in patients with glaucoma who require additional reduction despite maximally tolerated therapy. It is also indicated to prevent or reduce increased intraocular pressure during and after certain types of laser eye surgery. Apraclonidine acts as a relatively selective alpha2-adrenergic receptor agonist, with weaker activity at alpha1 receptors. Its mechanism involves lowering intraocular pressure by reducing aqueous humor production and increasing uveoscleral outflow, likely through constriction of afferent ciliary process vessels. Additionally, apraclonidine is used diagnostically in Horner's syndrome due to its unique pharmacological profile.
