Apraglutide is a next-generation, long‐acting synthetic peptide analog of glucagon-like peptide‐2 (GLP‐2), designed to selectively activate the GLP‐2 receptor in the gastrointestinal tract. As a full agonist at this receptor, it stimulates intestinal growth, increases fluid and nutrient absorption, promotes healing, and improves epithelial integrity and function. Apraglutide is engineered for increased resistance to enzymatic degradation (notably by dipeptidyl peptidase‐4), resulting in a prolonged elimination half-life compared to native GLP‐2 and other analogs like teduglutide. Its primary indication is short bowel syndrome with intestinal failure (SBS‑IF), where it aims to reduce dependence on parenteral support by enhancing absorptive capacity. It is also being investigated for gastrointestinal graft-versus-host disease (GVHD). The drug has demonstrated positive results in Phase III trials for SBS‑IF and offers the advantage of once-weekly subcutaneous administration due to its extended half-life[1][4][5][6][7].
