APRIL-BAFF-Bicephali CAR-T is an investigational chimeric antigen receptor (CAR) T-cell therapy being developed for the treatment of relapsed or refractory multiple myeloma. This "bicephalic" (dual-targeting) construct utilizes binding domains derived from the ligands APRIL (A Proliferation-Inducing Ligand) and BAFF (B-cell Activating Factor). By leveraging these ligands, the CAR-T cells are designed to simultaneously target several receptors on the surface of malignant plasma cells, including B-cell maturation antigen (BCMA), transmembrane activator and CAML interactor (TACI), and the BAFF receptor (BAFF-R). This multi-antigen targeting strategy aims to overcome resistance mechanisms like antigen escape, which can lead to relapse in patients treated with single-target CAR-T therapies. The therapy involves the ex vivo genetic modification of a patient's T cells followed by reinfusion after lymphodepleting chemotherapy to induce a robust anti-tumor immune response.
