Aprotinin is a polypeptide and broad-spectrum serine protease inhibitor originally derived from bovine lung. It is primarily used to reduce perioperative blood loss and the need for blood transfusion in patients undergoing complex surgeries such as cardiopulmonary bypass during coronary artery bypass graft surgery. Aprotinin acts by inhibiting several serine proteases, including trypsin, chymotrypsin, plasmin, and kallikrein. Its inhibition of kallikrein leads to reduced formation of factor XIIa, thereby suppressing both the intrinsic coagulation pathway and fibrinolysis. By also inhibiting plasmin, aprotinin slows down fibrinolysis independently. These actions result in decreased bleeding risk during surgery by maintaining hemostatic balance without increasing thrombosis risk.
