Apunemin (also known as CPT or Circularly Permuted TRAIL) is a recombinant protein and a variant of the human tumor necrosis factor-related apoptosis-inducing ligand (TRAIL). It is designed as a pro-apoptotic agent that selectively binds to and activates death receptor 4 (DR4/TNFRSF10A) and death receptor 5 (DR5/TNFRSF10B) on the surface of cancer cells, triggering the extrinsic apoptotic pathway. Developed by Beijing Sunho Biopharmaceutical (a subsidiary of Beijing SL Pharmaceutical), apunemin features a circularly permuted structure intended to enhance its stability and anti-tumor potency compared to native TRAIL. In China, it is approved under the brand name Risento for the treatment of relapsed or refractory multiple myeloma in combination with thalidomide and dexamethasone. It is also undergoing clinical evaluation for various other malignancies, including bone and soft tissue sarcomas. Note: In some clinical trial registries (e.g., ChiCTR2500113769), the name "Apunemin" has been associated with a Bcl-2 inhibitor, likely due to a naming error in the registry entry where it should have referred to lisaftoclax (APG-2575).
