Overview

**AQ4N + temozolomide** is a combination therapy under clinical investigation primarily for the treatment of newly diagnosed glioblastoma multiforme. AQ4N (banoxantrone) is a hypoxia-activated prodrug that, under low-oxygen conditions typical of solid tumors, is enzymatically reduced to AQ4, which functions as a DNA topoisomerase II inhibitor, selectively killing hypoxic tumor cells. Temozolomide is an oral alkylating agent that methylates DNA and is standard of care in glioblastoma, primarily targeting normoxic tumor cells. Combining the two agents aims to enhance overall antitumor efficacy by targeting both hypoxic and normoxic regions of tumors. Clinical trials have evaluated the safety, tolerability, and efficacy of this combination, often together with radiotherapy.
