Aqueous cream is a standard oil-in-water emulsion emollient widely used in clinical practice as a moisturiser and soap substitute. It is a staple in the management of atopic dermatitis and the prevention of radiation-induced skin reactions, such as erythema and desquamation, in patients undergoing radiotherapy for breast cancer. The formulation typically consists of liquid paraffin, white soft paraffin, and emulsifying wax. While traditionally formulated with sodium lauryl sulfate (SLS) as an emulsifier, clinical evidence suggesting that SLS can damage the skin barrier has led to the increased use of SLS-free versions. In clinical trials, aqueous cream often serves as the standard-of-care control for evaluating the efficacy of newer topical agents and barrier-forming dressings.
