The aqueous extract of Salvia miltiorrhiza Bge. f. alba (AESM) is a botanical preparation derived from the white-flowered variant of the traditional Chinese medicinal herb *Salvia miltiorrhiza* (Danshen). It contains water-soluble bioactive compounds such as salvianolic acids and danshensu. Research, primarily conducted by academic institutions like the Shandong University of Traditional Chinese Medicine, has focused on its potential therapeutic effects in pulmonary hypertension (PH). In preclinical rat models, AESM has demonstrated the ability to reduce mean pulmonary artery pressure, improve right ventricular function, and attenuate vascular remodeling. Its mechanism of action involves protecting the vascular endothelium from injury, modulating the balance of vasoactive substances (increasing nitric oxide and prostacyclin while decreasing endothelin-1 and thromboxane A2), and reducing levels of transforming growth factor-beta 1 (TGF-β1).
