AR-2126 is a novel siderophore-cephalosporin small molecule antibiotic being developed by ARIVA Med. It is designed to treat severe infections caused by multi-drug resistant Gram-negative bacteria, including *Acinetobacter baumannii*, *Pseudomonas aeruginosa*, and the *Burkholderia complex*. The drug utilizes a 'Trojan Horse' mechanism, where the siderophore component exploits the bacterial iron-uptake system to actively transport the molecule across the outer membrane. Once inside the periplasm, the cephalosporin component inhibits cell wall synthesis by targeting penicillin-binding proteins. AR-2126 is currently in the IND-enabling phase of development for the treatment of hospital-acquired bacterial pneumonia (HABP) and ventilator-associated bacterial pneumonia (VABP), with formulations intended for both intravenous and inhalation administration.
