Overview

Arazasetron besylate (SENS-401) is an orally bioavailable small molecule 5-HT3 receptor antagonist and calcineurin inhibitor being developed by Sensorion for the treatment and prevention of sensorineural hearing loss. SENS-401 is the (R)-enantiomer of arazasetron, a compound originally developed by Mitsubishi Tanabe Pharma for gastrointestinal disorders but later repurposed for its potent otoprotective properties. The drug is designed to protect the delicate hair cells and neurons of the inner ear from damage caused by various insults, including cisplatin chemotherapy, acoustic trauma, and surgical trauma from cochlear implantation. By antagonizing 5-HT3 receptors and inhibiting calcineurin in the cochlea, it is thought to mitigate excitotoxicity, inflammatory responses, and apoptotic pathways that lead to permanent hearing loss. It is currently in Phase 2 clinical trials for sudden sensorineural hearing loss (SSNHL), cisplatin-induced hearing loss (CIHL), and for preserving hearing in patients undergoing cochlear implantation.
