Drug intelligence / Profile preview
Argatroban and alteplase represent a combination therapy that has been studied for the treatment of acute ischemic stroke. This combination pairs argatroban, a direct thrombin inhibitor, with alteplase, a tissue plasminogen activator (tPA) that serves as a thrombolytic agent. The research on this combination has shown mixed results. While some earlier studies suggested potential safety and warranted further evaluation, more recent research indicates that adding argatroban to alteplase thrombolysis may not provide significant advantages for acute stroke patients. A meta-analysis examining this combination found that for the primary outcome of 90-day functional independence (modified Rankin scale score ≤2), the risk ratio for patients using argatroban after alteplase thrombolytic therapy compared to those not using argatroban was 1.00 (95% CI, 0.92-1.09; P = 0.97), indicating no statistical significance. In contrast, the combination of argatroban with antiplatelet therapy has shown more promising results. ## Mechanism of Action Argatroban works as a direct, selective thrombin inhibitor that can prevent the formation of thrombi. It has a short half-life. Alteplase functions as a tissue plasminogen activator that breaks down blood clots by converting plasminogen to plasmin, which then degrades fibrin in clots. The theoretical basis for combining these medications is that while alteplase works to break down existing clots, argatroban may help prevent reocclusion and further clot formation by inhibiting thrombin. ## Safety Profile The combination therapy does not appear to significantly increase the risk of hemorrhagic events compared to not using argatroban (RR, 1.08 [95% CI, 0.88-1.33]; P = 0.46). However, caution is warranted as the combination could potentially increase bleeding risk. ## Current Status Based on the available research, further studies are needed to definitively evaluate the effects of this combination treatment, particularly in the context of acute ischemic stroke. Current evidence suggests that the combination of argatroban with antiplatelet agents may be more promising than its combination with alteplase.
