Overview

Arikayce (amikacin liposome inhalation suspension) is an aminoglycoside antibiotic developed by Insmed Incorporated, specifically formulated for pulmonary delivery via a nebulizer. The drug consists of amikacin encapsulated within neutral charge liposomes, which facilitates the penetration of the antibiotic into bacterial biofilms and alveolar macrophages while minimizing systemic absorption and associated side effects like nephrotoxicity and ototoxicity. Its primary mechanism of action involves binding to the 30S ribosomal subunit of susceptible bacteria, thereby disrupting protein synthesis and leading to cell death. While Arikayce is FDA-approved for the treatment of refractory *Mycobacterium avium* complex (MAC) lung disease, it has also been evaluated in Phase 2 clinical trials for the management of chronic *Pseudomonas aeruginosa* infections in patients with non-cystic fibrosis bronchiectasis.
