aripiprazole + clomipramine

Aripiprazole + clomipramine is a non-fixed, off-label pharmacological combination used primarily in psychiatric practice for treatment-resistant cases of major depressive disorder (MDD) and obsessive-compulsive disorder (OCD). Aripiprazole is an atypical antipsychotic that acts as a partial agonist at dopamine D2 and serotonin 5-HT1A receptors, and an antagonist at serotonin 5-HT2A receptors. Clomipramine is a tricyclic antidepressant (TCA) with potent inhibition of serotonin reuptake and moderate inhibition of norepinephrine reuptake. The rationale for combining these agents lies in their complementary mechanisms: clomipramine addresses serotonergic dysfunction while aripiprazole modulates dopaminergic and additional serotonergic pathways. This strategy has shown efficacy in patients who do not respond to monotherapy with either agent, particularly in severe or refractory depression and OCD[1][2][3][7]. There are no commercially available products containing both drugs; the combination is achieved by co-prescribing individual agents.
