Aroxybutynin, also known as (R)-Oxybutynin, is a novel antimuscarinic and the R-enantiomer of oxybutynin. It exhibits antispasmodic, antimuscarinic, and anticholinergic activities, primarily functioning as a muscarinic receptor antagonist. While oxybutynin is used to treat overactive bladder, aroxybutynin is being specifically developed by Apnimed as a component of AD109, a once-daily oral medication for obstructive sleep apnea (OSA). In this combination, aroxybutynin works synergistically with atomoxetine to enhance upper airway muscle activity during sleep, aiming to prevent airway collapse in OSA patients. It is hypothesized to offer a safer side effect profile, particularly regarding urinary issues, compared to the racemic oxybutynin.
