arTreg-CSB is an investigational adoptive cellular therapy consisting of donor alloantigen-specific CD4+ regulatory T cells (Tregs) expanded ex vivo using costimulatory blockade. Developed by the National Institute of Allergy and Infectious Diseases (NIAID), it is designed to induce operational tolerance in liver transplant recipients, potentially allowing for the withdrawal of standard immunosuppressive medications. The therapy involves isolating Tregs from the recipient and expanding them in the presence of donor-derived cells and costimulatory blocking agents to create a population of cells specifically reactive to the donor's alloantigens. It is currently being evaluated in Phase I/II clinical trials.
