AS101 + carboplatin + etoposide is an investigational combination therapy comprising three agents: AS101 (ammonium trichloro(dioxoethylene-O,O')tellurate), carboplatin, and etoposide. AS101 is a small molecule immunomodulator and redox-active tellurium compound with anti-tumor, chemoprotective, radioprotective, and anti-inflammatory properties. It acts by modulating immune responses, increasing aldehyde dehydrogenase activity in bone marrow progenitor cells (which confers protection against chemotherapy-induced toxicity), inhibiting cysteine proteases such as cathepsin B, downregulating survival proteins like survivin, inhibiting the Akt pathway, and reducing tumor interleukin-10 production[1][2][3][4][5][9]. Carboplatin is a platinum-based chemotherapeutic agent that induces DNA crosslinking leading to apoptosis of rapidly dividing cells. Etoposide is a topoisomerase II inhibitor that causes DNA strand breaks during replication. The combination aims to enhance antitumor efficacy while reducing myelosuppression and other toxicities associated with chemotherapy by leveraging the protective effects of AS101 on normal hematopoietic cells[1][4]. Preclinical studies have shown synergistic antitumor effects when combining AS101 with various chemotherapeutics; clinical trials have explored its use in cancer patients for both direct anticancer activity and mitigation of chemotherapy side effects[6][7][8].
