Overview

BGE-175 (asapiprant) is an orally administered small molecule drug that acts as a selective antagonist of the prostaglandin D2 (PGD2) DP1 receptor. It was originally discovered by Shionogi and has been developed further by BioAge Labs for the treatment of age-related immune dysfunction, particularly in older adults hospitalized with COVID-19. The drug works by inhibiting the PGD2-DP1 signaling pathway, which becomes more active with age and impairs immunity through reduced dendritic cell migration and increased neutrophil-driven inflammation. By blocking this pathway, BGE-175 restores immune function to a more youthful state—boosting dendritic cell activity and reducing harmful inflammation—thereby improving host defense against viral infections such as SARS-CoV-2. Clinical trials have shown it to be well tolerated and safe in over 2,400 subjects for other indications such as allergic rhinitis. As of 2021–2022, BGE-175 is being evaluated in Phase 2 clinical trials for its ability to prevent respiratory failure and mortality in older patients hospitalized with COVID-19[1][3][6][7].
