ascorbic acid + thiamine + hydrocortisone is an investigational combination therapy being evaluated by Asan Medical Center in the STAR (Steroid, Thiamine and Ascorbic Acid in Out-of-hospital Cardiac Arrest) trial. The regimen is designed to treat post-cardiac arrest syndrome in comatose survivors of out-of-hospital cardiac arrest (OHCA) who are undergoing targeted temperature management. The therapy aims to attenuate the whole-body ischemia/reperfusion injury and systemic inflammatory response that occurs following the return of spontaneous circulation. Ascorbic acid (Vitamin C) provides antioxidant support, thiamine (Vitamin B1) acts as a cofactor for pyruvate dehydrogenase to improve mitochondrial function, and hydrocortisone (cortisol) provides anti-inflammatory effects and addresses potential adrenal insufficiency. The combination is administered intravenously every 12 hours for three days.
