Ascorbic acid (Vitamin C) is a small molecule antioxidant and essential cofactor being investigated by Mashhad University of Medical Sciences as a component of a combination therapy for non-healing diabetic foot ulcers. In this therapeutic regimen, oral ascorbic acid is administered in conjunction with oral vitamin E and a topical platelet-rich plasma-fibrin glue (PRP-FG) dressing. Ascorbic acid facilitates wound healing by reducing oxidative stress—which is typically elevated in diabetic patients and impairs tissue repair—and by serving as a necessary cofactor for prolyl hydroxylase in collagen synthesis. A Phase 2/3 clinical trial (NCT04315909) conducted by the university demonstrated that the addition of these antioxidant vitamins to PRP-FG dressing significantly improved wound closure rates and reduced oxidative markers in patients with chronic diabetic ulcers.
