ASCOT (Autologous Stem Cell Ovarian Transplantation) is an investigational cell therapy developed by the Instituto de Investigación Sanitaria La Fe (IIS La Fe) in collaboration with IVIRMA Global. The procedure involves the mobilization of autologous bone marrow-derived stem cells (BMDSCs) into the peripheral blood using granulocyte-colony stimulating factor (G-CSF). These cells are then collected via apheresis and infused directly into the ovarian artery. The therapy is designed to treat premature ovarian failure (POF) and premature ovarian insufficiency (POI) by promoting the reactivation of dormant follicles and improving the ovarian microenvironment. Clinical studies, including the Phase 4 ASCOT-2 trial, have demonstrated that this approach can restore ovarian function, as evidenced by increased levels of Anti-Müllerian Hormone (AMH) and antral follicle counts, ultimately facilitating spontaneous pregnancies and live births in women who previously had few options other than egg donation.
