Overview

This fixed-dose combination (FDC) polypill, developed by the Isfahan University of Medical Sciences and manufactured by Alborz Darou, is a small molecule therapy designed for the secondary prevention of major cardiovascular events in patients following acute myocardial infarction. The polypill contains four active ingredients: aspirin (81 mg), atorvastatin (40 mg), metoprolol succinate (47.5 mg), and valsartan (40 mg). Aspirin acts as an antiplatelet agent by irreversibly inhibiting cyclooxygenase (COX) enzymes. Atorvastatin is an HMG-CoA reductase inhibitor used to lower lipid levels. Metoprolol succinate is a selective beta-1 adrenergic receptor antagonist that reduces heart rate and blood pressure, while valsartan is an angiotensin II receptor type 1 antagonist that further aids in blood pressure management and cardiovascular protection. The formulation was evaluated in the Persian Polypill Study (NCT03541109), a multicentric, randomized clinical trial which demonstrated that the polypill strategy significantly improved medication adherence and patient satisfaction compared to usual care in patients hospitalized with ST-elevation myocardial infarction (STEMI) or non-ST elevation myocardial infarction (NSTEMI).
