**aspirin + BMS-986141** is an experimental oral combination therapy of **acetylsalicylic acid (aspirin)**, an established nonsteroidal anti-inflammatory drug (NSAID) and antiplatelet agent, with **BMS-986141**, a potent, selective, reversible small molecule antagonist of protease-activated receptor 4 (PAR4). BMS-986141 targets PAR4-mediated platelet activation, a late-stage process in thrombus formation, to reduce platelet aggregation and thrombus growth, with the aim of providing additional antithrombotic effect to standard-of-care agents like aspirin. The combination has been evaluated primarily for its antithrombotic effects in patients with stable coronary artery disease, as well as in stroke prevention trials, with the rationale that PAR4 inhibition may offer strong antithrombotic efficacy with a lower risk of bleeding than other regimens. BMS-986141 has demonstrated selective inhibition of PAR4-induced platelet aggregation and ex vivo thrombus formation, with additive effects when combined with aspirin. Clinical development for BMS-986141 as a standalone and in combination with aspirin has been discontinued after early phase trials.
