Aspirin + butalbital + caffeine + codeine is an oral fixed-dose **small molecule combination** analgesic containing aspirin, butalbital, caffeine, and codeine phosphate. It is FDA-approved in capsule form for the management of the symptom complex of **tension headache** or **muscle contraction headache** when non-opioid analgesics and alternative treatments are inadequate. The combination pairs aspirin-mediated cyclooxygenase inhibition with barbiturate sedative effects from butalbital, adenosine receptor antagonism and analgesic adjuvant activity from caffeine, and opioid receptor agonism from codeine. Marketed in the United States under brand names including Ascomp with Codeine, Fiorinal with Codeine, and Fiortal with Codeine, it is reserved for patients who have not responded adequately to other options because of risks including addiction, abuse, misuse, overdose, and life-threatening respiratory depression related mainly to the butalbital and codeine components.
