aspirin + dipyridamole + prednisone

This is a combination of three drugs—aspirin, dipyridamole, and prednisone. Aspirin is an antiplatelet agent that irreversibly inhibits cyclooxygenase-1 (COX-1), reducing thromboxane A2 production and thereby inhibiting platelet aggregation. Dipyridamole is a phosphodiesterase inhibitor that increases intracellular cyclic AMP in platelets, further inhibiting platelet aggregation and also has vasodilatory properties[3][6]. Prednisone is a synthetic glucocorticoid with potent anti-inflammatory and immunosuppressive effects; it acts by modulating gene expression to suppress pro-inflammatory cytokines and mediators[1]. While the combination of aspirin and dipyridamole (marketed as Aggrenox) is approved for secondary stroke prevention[2][3][5], the addition of prednisone represents an investigational or off-label approach. Preclinical studies suggest that combining prednisolone (a closely related corticosteroid) with dipyridamole may enhance anti-inflammatory effects while potentially reducing glucocorticoid-associated adverse events at lower doses[1].
