Overview

Aspirin + P2Y12 antagonist is a combination drug regimen commonly referred to as dual antiplatelet therapy (DAPT). This regimen combines aspirin, an irreversible cyclooxygenase-1 (COX-1) inhibitor that suppresses thromboxane A2-mediated platelet activation, with a P2Y12 receptor antagonist (such as clopidogrel, prasugrel, ticagrelor, or cangrelor), which blocks the ADP-mediated activation of platelets. The combination provides synergistic inhibition of platelet aggregation and is more effective than either agent alone in reducing the risk of arterial thrombosis. DAPT is primarily used for secondary prevention in patients with acute coronary syndromes (ACS), after percutaneous coronary intervention (PCI) with stent placement, and for prevention of recurrent stroke or other atherothrombotic events. The choice of specific P2Y12 antagonist depends on clinical context and patient characteristics[1][3][5][7].
