Aspirin + ticagrelor or aspirin + clopidogrel

Aspirin combined with either ticagrelor or clopidogrel is a dual antiplatelet therapy (DAPT) regimen widely used to prevent thrombotic cardiovascular events in patients with acute coronary syndromes (ACS), including unstable angina and myocardial infarction. Aspirin acts as an irreversible inhibitor of cyclooxygenase-1 (COX-1), reducing thromboxane A2 production and thus platelet aggregation. Ticagrelor and clopidogrel are P2Y12 receptor antagonists that inhibit ADP-mediated platelet activation; however, ticagrelor is a direct-acting reversible inhibitor while clopidogrel is an irreversible prodrug requiring metabolic activation[6][7]. The combination of these agents provides synergistic inhibition of platelet aggregation to reduce the risk of heart attack, stroke, and vascular death[5][7]. Ticagrelor plus aspirin has demonstrated superior efficacy compared to clopidogrel plus aspirin in reducing major adverse cardiovascular events but may be associated with higher bleeding risk[2][5].
