This combination therapy consists of aspirin, a nonsteroidal anti-inflammatory drug (NSAID), and zileuton, a 5-lipoxygenase inhibitor. Developed by the National Cancer Institute (NCI) for lung cancer prevention, the regimen aims to inhibit both the cyclooxygenase (COX) and 5-lipoxygenase (5-LOX) pathways of arachidonic acid metabolism. By reducing the production of pro-inflammatory prostaglandins and leukotrienes, the combination is hypothesized to modulate tobacco-related gene expression signatures and reduce the risk of lung carcinogenesis in current smokers. A Phase II trial (NCT02348203) evaluated this combination's effect on biomarkers in the nasal and bronchial epithelium, showing favorable modulation of a bronchial gene expression signature associated with squamous dysplasia.
