Aspirin mouthwash is a topical formulation of acetylsalicylic acid developed by the Institute of Head and Neck Studies and Education (InHANSE) in the United Kingdom for the treatment of oral leukoplakia and oral dysplasia. As a small molecule non-steroidal anti-inflammatory drug (NSAID), aspirin irreversibly inhibits cyclooxygenase enzymes (COX-1 and COX-2), which are involved in the synthesis of prostaglandins that promote inflammation and cell proliferation in precancerous oral tissues. The treatment involves dissolving aspirin tablets (75 mg to 300 mg) in water to be used as a gargle, aiming to provide high local concentrations of the drug to the oral mucosa while minimizing systemic side effects. A Phase I trial was conducted to assess safety and biomarker modulation, but it was terminated due to low patient recruitment.
