Overview

211At-SubP is a targeted alpha-particle therapy (TAT) radiopharmaceutical consisting of the neuropeptide Substance P labeled with the alpha-emitting radioisotope astatine-211. Developed by the ACCELERATE.EU consortium, the compound is designed to treat glioblastoma multiforme (GBM) by targeting the neurokinin-1 receptor (NK1R), which is highly expressed on the surface of glioma cells but has limited expression in healthy brain tissue. Upon binding to NK1R, the astatine-211 isotope delivers high-linear energy transfer (LET) alpha radiation over a very short range (50-100 μm), causing lethal double-strand DNA breaks in the tumor cells while sparing adjacent healthy neurons. The drug is typically administered via local injection into the tumor or the post-operative resection cavity to bypass the blood-brain barrier and maximize local dose intensity.
