Astragalus + gemcitabine is an investigational combination therapy for the neoadjuvant treatment of pancreatic cancer. It combines Astragalus polysaccharide (APS), a traditional Chinese medicine preparation from *Astragalus membranaceus*, with the standard chemotherapy drug gemcitabine. APS is an immunomodulator and biologic agent that is believed to enhance anticancer immune responses, partly by modulating the PI3K/Akt/mTOR pathway. [14, 17, 18, 19] Clinical studies suggest that the combination therapy improves objective response rates, disease control rates, and median survival time in pancreatic cancer patients compared to gemcitabine alone, while also reducing chemotherapy-related side effects like leukopenia and fatigue.
