Astragalus + notoginseng granule is a traditional Chinese medicine (TCM) herbal granule formulation used for the treatment of diabetic nephropathy (DN), particularly in patients presenting with a TCM syndrome pattern of qi deficiency and blood stasis. It is a standardized, boil-free herbal extract granule preparation whose primary active ingredients are derived from *Astragalus* (Huangqi; *Astragalus mongholicus* Bunge or *Astragalus membranaceus*) and *Panax notoginseng* (Sanqi), and may also include supporting herbs such as *Angelica sinensis*, *Achyranthes bidentata*, and *Ecklonia kurome*. The formulation is proposed to target the Mincle/Card9/NF-κB signaling pathway (involved in macrophage-mediated renal inflammation) and the mTOR/PINK1/Parkin autophagy pathway. It is being evaluated in a multicenter Phase 4 randomized parallel-group clinical trial in China, sponsored by Shanghai Sixth People's Hospital East, in combination with losartan to assess improvements in proteinuria, podocyte-related markers, and renal function in adults with early diabetic nephropathy.
