Astragalus granules are a standardized herbal formulation derived from the root of *Astragalus membranaceus* (Huangqi), a cornerstone of Traditional Chinese Medicine (TCM). In oncology, it is primarily utilized as an adjuvant therapy to enhance the therapeutic index of standard treatments such as chemotherapy and immune checkpoint inhibitors (e.g., PD-1 inhibitors). The preparation is rich in astragalus polysaccharides (APS), flavonoids, and saponins, which are thought to exert immunomodulatory effects by promoting T-cell activation, regulating cytokine production, and potentially modulating the intestinal microbiota to improve the host's anti-tumor immune response. A Phase 4 randomized trial led by Beijing Friendship Hospital is currently evaluating the efficacy of oral astragalus granules in combination with PD-1 inhibitors and chemotherapy for patients with advanced gastric cancer.
