AT-MSC secretome refers to the total collection of bioactive molecules released by adipose tissue-derived mesenchymal stem cells (AT-MSCs) into their surrounding environment. This complex mixture includes soluble proteins (such as growth factors and cytokines), extracellular vesicles (including exosomes and microvesicles), lipids, nucleic acids (like miRNAs), and other signaling molecules. The therapeutic rationale is that these components mediate many of the regenerative, anti-inflammatory, immunomodulatory, neuroprotective, anti-fibrotic, angiogenic, and antioxidant effects attributed to MSCs themselves[1][3][5][7]. Instead of transplanting live cells—which carries risks such as immune rejection or tumorigenicity—secretome therapy delivers only the cell-free products for a potentially safer treatment. The composition can be tailored by preconditioning MSCs in vitro to enhance specific therapeutic properties[5]. Clinical applications under investigation include chronic inflammatory diseases (e.g., rheumatoid arthritis), neurological disorders (e.g., multiple sclerosis, ALS), musculoskeletal injuries or degeneration (e.g., osteoarthritis), spinal cord injury repair, erectile dysfunction/impotence, infertility treatments[1][8], anti-aging interventions[1], muscle diseases[6], retinal degenerative diseases[2], traumatic brain injury[9], and more.
