This is a combination regimen consisting of **atacicept** (a fully human recombinant fusion protein targeting B-cell activating factors), **mycophenolate mofetil** (an immunosuppressive agent that inhibits lymphocyte proliferation by blocking purine synthesis), and **corticosteroids** (a class of anti-inflammatory and immunosuppressive hormones). Atacicept inhibits the action of BLyS (B-Lymphocyte Stimulator) and APRIL (A proliferation-inducing ligand), thus suppressing B-cell activation and antibody production; mycophenolate mofetil inhibits inosine monophosphate dehydrogenase, reducing guanosine nucleotide synthesis and thus T and B cell proliferation; corticosteroids modulate gene expression to suppress inflammation and multiple immune pathways. The primary indication for this combination was active lupus nephritis (LN), a severe kidney complication in systemic lupus erythematosus (SLE). Trials combining these agents were terminated early due to increased risk for severe hypogammaglobulinemia and serious infections[1][3][5]. Atacicept was developed by ZymoGenetics/EMD Serono (Merck Serono) and operational oversight was by ZymoGenetics.
