This is a combination of four agents studied in research settings to investigate and potentially counteract the drug-drug interaction between rabeprazole (a proton pump inhibitor) and atazanavir/ritonavir (an antiretroviral medication combination). Atazanavir/ritonavir is used to treat HIV infection. Rabeprazole significantly reduces gastric acid, which impairs the absorption of atazanavir, as it requires an acidic environment for optimal uptake. Betaine hydrochloride has been explored as a potential agent to re-acidify the stomach and restore atazanavir absorption when co-administered with rabeprazole. Studies have shown that rabeprazole substantially decreases atazanavir exposure (around 70%). The addition of betaine hydrochloride in these studies only partially restored atazanavir levels (approximately 12-13% restoration). This specific four-drug combination is not a commercially available product but represents a research approach to managing complex drug interactions in patient care.
