ATB-428 is an investigational hydrogen sulfide (H2S)-releasing derivative of mesalamine being developed by Antibe Therapeutics for the treatment of inflammatory bowel diseases such as Crohn’s disease and ulcerative colitis.[1][9][13] It consists of mesalamine covalently linked to the H2S-donor 4-hydroxythiobenzamide, creating a small-molecule conjugate that delivers both the established 5-aminosalicylic acid anti-inflammatory activity and additional cytoprotective, anti-inflammatory, and microbiome-modulating effects attributed to controlled H2S release.[1] Preclinical work indicates that H2S-releasing mesalamine analogs like ATB-428 can scavenge intestinal iron and attenuate virulence of colitis-associated microbiota, thereby reducing mucosal injury and inflammation and supporting mucosal healing, positioning this compound as a next-generation IBD therapy that targets both host inflammation and microbiota-driven pathology.[1][7][9][13]
