Overview

This is a combination of multiple cardiovascular medications including five beta-blockers (atenolol, bisoprolol, metoprolol, nebivolol, propranolol), two calcium channel blockers (diltiazem, verapamil), and one cardiac glycoside (digoxin). These medications are commonly used to treat various cardiovascular conditions including hypertension, heart failure, atrial fibrillation, and angina. **Beta-blockers** (atenolol, bisoprolol, metoprolol, nebivolol, propranolol) work by blocking the action of the sympathetic nervous system, reducing heart rate and blood pressure. **Calcium channel blockers** (diltiazem, verapamil) inhibit calcium movement into cardiac and smooth muscle cells, causing vasodilation and decreased heart rate. **Digoxin** increases the force of heart muscle contractions by inhibiting the sodium-potassium ATPase enzyme, which increases intracellular calcium. It also has parasympathetic effects that slow conduction at the AV node. Combination therapy using these medications must be carefully monitored as there are significant potential interactions. Beta-blockers combined with calcium channel blockers can cause additive effects on heart rate and blood pressure. Digoxin combined with beta-blockers can cause excessive bradycardia. Recent research suggests that combination therapy of beta-blockers and digoxin may be associated with increased risk of major adverse cardiovascular events and all-cause mortality compared to beta-blockers alone.
