Overview

Atezolizumab + obinutuzumab is a combination of two monoclonal antibody therapies investigated for the treatment of hematologic malignancies, primarily chronic lymphocytic leukemia (CLL), follicular lymphoma (FL), and mantle cell lymphoma (MCL)[5][6][3][1]. **Atezolizumab** is a humanized IgG monoclonal antibody that targets programmed death-ligand 1 (PD-L1), preventing its interaction with PD-1 and B7-1, thereby lifting inhibitory signals on T cells to restore their cytotoxic function against cancer cells[2][6]. **Obinutuzumab** is a type II anti-CD20 monoclonal antibody that binds to the CD20 antigen on B lymphocytes, leading to B-cell depletion via enhanced antibody-dependent cytotoxicity and direct cell death[4][6]. Both molecules are produced by recombinant DNA technology. The combination has demonstrated immunomodulatory and antineoplastic activity in phase Ib/II clinical trials, but with notable adverse events such as immune-related toxicity and hematologic complications[6][1][5]. The regimen has been studied both as a backbone and in combination with other agents (e.g., bendamustine, venetoclax)[1][6]. Primary indications under investigation are previously untreated or relapsed/refractory CLL, FL, MCL, and Waldenström's macroglobulinemia[5][6][1].
